If so, you need a doctor to certify the death. The Doctor can be your own family doctor or we can provide one for you that is on our list of 24 hour doctor services.

Occured at the hospital

a) The Doctor at the hospital can fill up the CCDO Form before passing to the ward nurse.
b) Family to get CCDO from ward nurse bringing along the Identity card of Deceased.

2. Engage the service of a funeral planner

A funeral planner will help you plan out all the rest of the funeral. This can be done by giving us a call and a planner would be send to you within 1/2hr for a no obligation consultation. From the discussion, we would find out what kind of funeral are you looking at and do our very to achieve what you want. After which we would do our ultimate best to take care of all logistic issue on your behalf.

3. Additional info for the funeral service

Booking of Void Deck

The booking of void deck has to be done yourself via HDB. You would need to bring the cause of death cert issued by the doctor or hospital to the police station to get a certificate of death. This is needed at your town council to do the booking for your void deck. Unfortunately, we are unable to assist in this step as it is required for the next of kin to be present.
