Step 1: Obtain Certificate of Cause of Death ( CCOD)
If Cause of death is natural and known: 1) The Doctor at the hospital can fill up the CCDO
Form before passing to the ward nurse
2) Family to get CCDO from ward nurse bringing along the Identity card of Deceased.
If cause of death can't be determined by doctor or has been contributed by unnatural event(eg:
surgical complication, a fall prior to admission):
1. Doctor will refer the case to the police/coroner as required by law
2. Body is than brought to Centre for Forensic Medicine (CFM) Mortuary located at Block 9 of Singapore General
Hospital(SGH) via police hearse.
3. The family will be told by the police when to go down to the CFM Mortuary, usually by the next day.
The family should bring along the following to the CFM:
1. All medical documents relating to the deceased.
2. All medicine consumed by the deceased
3. Identification papers of the deceased such as NRIC/Passport/Birth Cert etc.
At the CFM, the police investigator will arrange for the family to view and identify the body of the deceased in
the presence of the coroner.
The coroner will than review the case and determine if a autopsy is required. The family will than be informed of
the coroner's decision and the time to claim the body for the funeral.
Note: If the autopsy reveals that the death is unnatural. the police will need to conduct further investigations
into the cause of death and the family will need to assist. Once the investigation is completed, the family will be
required to attend a coroner inquiry at the subordinate courts.
